When a student is struggling, you need a plan you can follow in minutes—not a stack of scattered forms. The Student Mental Health Response Toolkit gives school counselors one clear, documented path from the first sign of concern to safety planning, family contact, referral, Tier 2 support, return-to-school, and follow-up data. We call it the Concern-to-Care System.

The problem this toolkit solves

Students outnumber school counselors about 372 to 1 nationally (ASCA, 2024–25), and roughly 4 in 10 high school students report persistent sadness or hopelessness (CDC YRBS, 2023). Most counselors are responding to serious mental health concerns with whatever forms they can find, their memory, and sticky notes. That leads to inconsistent responses, missed family follow-ups, referrals that never connect, and students returning from a crisis with no plan.

This toolkit replaces that scramble with a repeatable system: the same careful steps, every time, for every student—with documentation that protects students, families, and you.

Why counselors choose it

One system, start to finish. Most resources cover one piece (a tracker, or a safety plan). This connects every step.
Fillable and printable. Type directly into the forms or print them—save a blank master and “Save As” for each student.
Grounded in professional guidance. Aligned with ASCA’s position on suicide prevention, intervention, and postvention: parent notification, safety planning, means-safety conversations, and re-entry meetings.
Admin-ready data. Turn your daily work into monthly and year-end numbers that show what your program does—and what it needs.

Frequently asked questions

Is this a suicide risk assessment?

No. This is an organizational and educational toolkit. It helps you follow your district’s protocol consistently and document it well. Use the screening instrument your district has approved (for example, the Columbia Protocol school screener or the ASQ) from its official source.

Can I edit the forms?

Yes—every form has fillable fields you can type into using a free PDF reader such as Adobe Acrobat Reader. You can also print any page.

Can I use it with all of my students?

Yes. Your single-user license covers every student, family, and staff member on your own caseload, every school year. For multiple counselors, please purchase one license per counselor.

What format is it?

One 44-page PDF, U.S. letter size, delivered as an instant download after purchase.
